Honesdale Office is Closing
Dr Epstein will no longer be seeing Outpatients as of June 30, 2025, and is closing the Honesdale office location on June 30, 2025. Current outpatients have been advised to follow up with their primary care providers after June 30, 2025.
Dr. Epstein will continue overseeing the Wayne Memorial Inpatient Rehabilitation Unit after June 30, 2025.
Dr. Epstein will continue to perform IME/IRE evaluations, depositions and other medical-legal work after June 30, 2025.
Medical Records
Effective July 20, 2025, we have retained Cariend, a medical records custodian, to manage the records for the next 7-15 years.
Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed outpatient practice.
Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by HIPAA.
